The Pennsylvania Dietetic Association offers a preprinted bill which itemizes and describes all services and fees with codes in a form that the accounting systems of health care providers use. A client can submit this superbill directly to their health care insurer for MNT reimbursement or the RD can submit this bill for payment of services, under the specifics for CMS and Private insurers that cover MNT for their clients.

Note: As of November 1, 2006, these nationally used forms are now updated with the latest ICD-9 codes including food allergies and five new categories of Morbid Obesity. They are available on a new 8 1/2" x 11" 2-ply form so you can invoice your clients directly and can be ordered plain or personalized.
The ICD 9 - CM Codes are International Classification of Disease Codes. These codes are utilized by Health Care Providers on billing forms in order to classify diagnostic and surgical procedures.
The American Dietetic Association suggests that RDs use Medical Nutrition Therapy (MNT) current procedural terminology (CPT) codes for billing purposes. For medical nutrition therapy assessment and/or intervention performed by a physician, Evaluation and Management or Preventive Medicine service codes may be used. If you choose to use a CPT, it is imperative that the note you write in the medical chart reflects the stated description of the CPT code. You will need a CPT codebook to use for reference so you can chart appropriately. Physician offices and hospitals have these references. Remember, independent practitioners should attach a physician prescription/referral with pertinent diagnoses to the bill when submitting it for collection. This is to avoid any misrepresentation of the RD as practicing medicine without a license.
For Medicare billing, you will need to check with CMS for specific guidelines and forms that must be submitted with the superbill for reimbursement. Currently, Medicare will reimburse for MNT for Diabetes and renal disease, pre-dialysis.
Each private insurance company has specific criteria for MNT provider status and reimbursement. Before submitting a superbill to an insurance company, it is suggested you contact their provider coordinator to determine if MNT services by the RD are a covered service.
