These nationally used forms are updated (as of 11/06) with the latest ICD-9 codes including food allergies and five new categories of Morbid Obesity.   They are available on an 8 1/2" x 11" 2-ply form so you can invoice your clients directly and can be ordered plain or personalized.

HOW TO ORDER

1. Select the correct amount of Superbills you wish to purchase and click "add to cart". 

2. When ready to process your order click checkout.  PADA uses Paypal to process your on-line payment. If you aren't a member of paypal- you will be walked you through steps to pay with your credit card.

3. Paypal will send an e-mail to both you and our management services office once your transaction is completed. Your Superbills will be shipped in approximately 7-10 business days. Personalized superbills will be shipped in approximately three weeks. 

Sales tax is applied to shipping and handling in addition to the merchandise total.  All sales are final.

#2) Provider Number

#3) Name & Address as you would like it to appear on your superbill
