CE Prior Approval Requests
For
Registered Dietitians and Dietetic
Technicians, Registered
The
Pennsylvania Dietetic Association represents nearly 4,000 food
and nutrition experts in our state. Thank you for considering
offering continuing education credits for our members at your
next professional meeting. Below are the steps to have your
program reviewed for continuing education credits.
Process:
Call
the PADAF office at
614-436-6211
and request the RC-3 form to
be sent to you or download it at: http://www.cdrnet.org/PDFs/RC-3.pdf
Complete
the form and send it with the following information to the address
listed in Step 5:

Two
copies of the professional program or outline (or bibliographies
if Journal Club).

The credentials of each speaker,
if not listed on the program.

The appropriate fee, listed in
the Fee Schedule below.

If you downloaded the RC-3 form,
make and send 3 copies of the completed,
signed RC-3 form. If you are using the 3-part carbonless
RC-3 form provided by our office, send all 3 copies with your
request for approval.

Mail payment (check made payable
to PADAF), RC-3 form and required information to:

Fee
Schedule :
$30
to review a single continuing education activity for 1-5 credits
$60
to review of a single continuing education activity for 6-10
credits
$80
to review of a single continuing education activity for 11 or
more credits
$20
additional charge for extension of a continuing education activity
approval or for each repeat of the same activity within a 1-year
period . (Please
include a copy of the RC-3 or RC-6 form returned to you after
the first submission with the CE Committee Use block information
filled in.)
$10
per session for journal clubs and study groups
$20
late fee for requests after the 4-week deadline.
Requests
received less than 2 weeks before the program may NOT be able
to be processed and will NOT be returned.
Every
effort will be made to process them, but added fees may be incurred.

make check payable to: PADAF
or Pennsylvania Dietetic Association Foundation . Fees
are non-refundable. Program approval requests will not be reviewed
until the fee is received.
Time
for Processing :
Please
allow 4 weeks for processing. Failure
to complete and include all information requested will result
in a delay and our inability to complete the review. Upon approval,
a yellow copy of the Request Form will be
returned with the prior approval reporting forms.
