CE Prior Approval Requests

For Registered Dietitians and Dietetic Technicians, Registered

 

The Pennsylvania Dietetic Association represents nearly 4,000 food and nutrition experts in our state. Thank you for considering offering continuing education credits for our members at your next professional meeting. Below are the steps to have your program reviewed for continuing education credits.   

 

Instructions for Continuing Education Prior Approval Requests:

1. Download the Commission on Dietetic Registration (CDR) Continuing Professional Education Prior Approval Request Form (RC-3 Form) at: http://www.cdrnet.org/PDFs/RC-3.pdf

2. Follow the directions on the form for “required documentation” including:

A. Title of the presentation

B. Learner educational objectives describing anticipated outcomes:

Example

As a result of this program, participants will be able to:

    •  Use motivational interviewing techniques and research to help parents, caregivers, and children achieve an sustain healthy behaviors
     
    • Initiate evidence-based healthy lifestyle behavior change programs
     

C. Timing outline including:

    1. Presentation time
    2. Discussion time (if applicable)
    3. Registration time
    4. Introduction/welcome time
    5. Coffee and meal breaks

D. Qualifications of speaker on topic(s) presented

3. Check number of CPEUs (left side of form) requested based on the following:

  • A presentation, discussion, or activity must be one full hour in length to qualify.  Fractions of an hour are rounded up after 30 minutes and down at 29 minutes.

4. Note CPEU level as 1, 2, or 3 based on descriptions on RC-3 form.

5. One of the following:

a. Copy of the professional program with a summary description of the presentation (s).

OR

b. Outline of professional program content

OR

c. Bibliographies (for journal club)

Include 2 copies of the above documentation and mail with the required fee (see fee schedule below) to:

Rates Effective October 1, 2009

$45 to review a single continuing education activity for 1-5 credits

$75 to review of a single continuing education activity for 6-10 credits

$95 to review of a single continuing education activity for 11 or more credits

$30 additional charge for extension of a continuing education activity approval or for each repeat of the same activity within a 1-year period .   (Please include a copy of the original signed and approved RC-3 form previously returned to you).

$20 per session for journal clubs and study groups

$40 late fee for requests after the 4-week deadline.

 

Please make check payable to: PADAF or Pennsylvania Dietetic Association Foundation . Fees are non-refundable. Program approval requests will not be reviewed until the fee is received.

Time Processing:

Please allow 4 weeks for processing. Failure to complete and include all information requested will result in a delay and our inability to complete the review.  If all documentation is not included, the packet will be returned for correction and re-submission. Upon approval, an approval letter will be sent to the designated contact person along with a copy of the signed CDR RC-3 form showing number of CPEUs approved. You will be responsible for providing participants with CPEU certificates.

Requests received less than 2 weeks before the program may NOT be able to be processed and will NOT be returned. Every effort will be made to process them, but added fees may be incurred. Please call the PADAF office for more information.
