YOU can sign up now to be licensed and you will be billed later. There are three basic ways to apply. Remember this is a legal document, so you need to access the application yourself!

1.  Apply on-line at http://www.dos.state.pa.us.  You must be in the Department of State's Home page or Welcome page to access the most direct link.  Click on  Become a licensed Health Care Professional, next click on Nursing, next click on Licensure Information and scroll down to Application for Dietitian-Nutritionist License and click to the application.  Or you can go directly to the following web address:

Downloading the application from the above website is the quickest and easiest way to apply.

2.      Obtain an application by telephone by calling the PA State Board of Nursing at 717-783-7142. When you receive the application by mail, fill out the necessary information and mail. This process is time consuming because it can take a minimum of 7-10 business days before you will actually get the application in the mail. Also the phone lines are very busy and hard to get through to make your request.

3.      Fax a request for an application to the PA State Board of Nursing at 717-783-0822. Include your name, address/personal information. Fill out and mail, DO NOT fax back. See #2 for time frame.
