Tips and Suggestions for Accessing the Licensure Application from the Website

Web access is the quickest way! The application
has been on-line since the end of September.
If you enter a link asking for a licensure
number you are in the wrong page and need to try again, you do not need
to enter any number.
Downloading time is dependent on your Internet
server and your computer system.
You must be able to download a pdf form on
your computer system in order to get the application off the website. Don't
know if you can download a pdf form? You will know when you can not get
anything downloaded or the information you do get you won't be able to
read. Suggestion go to the library and get access to the web.
The application can not be downloaded into
a Word document. This is a legal form and can not be altered in any way.
