Position Description:
• Complete comprehensive nutritional assessments of the in center and home patients • Participates with the Interdisciplinary team in the development of a written, individualized plan of care for each patient.
• Develops appropriate interventions for each patient that allows them to meet their individualized nutritional goals. Provide ongoing monitoring of the interventions, revising them as needed to promote positive nutritional outcomes.
• Provides ongoing patient specific detailed education to the patient, family and/or caregiver presented in a way that considers their specific cognitive ability. These education approaches will be designed to target each patient’s individual learning needs.
• Completes progress notes at least monthly in accordance with State and Federal regulations.
• Collaborates with the Interdisciplinary team to track Mineral Bone Disease laboratory data and make appropriate recommendations based on the unit approved protocol.
• Gathers and reports QAPI data and participates in the monthly QAPI meetings.
• Develops action plans when needed to improve nutritional outcomes in the facility or to target nutritional problems identified by the interdisciplinary team.
• Works in conjunction with the Social Worker to ensure the patient/family has access to patient assistance programs for nutrition related medications or food assistance programs.
• Stays abreast of current topics in renal nutrition in order to maintain skills and knowledge to provide appropriate care of the patient.
• Attends training and maintains HIPPA compliance at all times.
• Other nutrition management duties as assigned.
Minimum Qualifications:
• Maintain Registration with the Commission on Dietetic Registration• Have a minimum of 1-year professional work experience in clinical nutrition as a registered dietitian.
• Must maintain a current Pennsylvania License.
• Maintains dietetic registration and licensure as required by Pennsylvania, including continuing education requirements as specified by the Commission on Dietetic Credentialing and the PA State Board of Nursing.
