We are seeking a dedicated and knowledgeable Dietitian to join our healthcare team. The ideal candidate will provide expert nutritional guidance, develop personalized care plans, complete MDS, and work with the wound and weight committees. This role involves assessing patient needs, implementing nutrition interventions, and collaborating with medical staff to promote optimal health outcomes. A strong background in dietetics, physiology, and healthcare systems is essential to succeed in this position.

16 hours/week onsite -CDR registered dietitian/nutritionist (RD/RDN)
-PA Licensed Dietitian-Nutritionist (LDN)
-Proven experience in dietetics within hospital or long-term care settings.
-Strong knowledge of physiology, nutrition science, and clinical care protocols.
-Experience supervising dietary staff and managing care plans effectively.
-Proficiency with EMR systems for accurate documentation and reporting.
-Skilled in health coaching techniques to motivate behavioral change for weight loss or lifestyle improvements.
-Familiarity with tube feeding procedures and acute care nutritional management.
-Ability to perform thorough patient assessments and diagnostic evaluations related to nutrition status.
-Experience working with diverse populations including geriatrics and patients requiring specialized diets such as those in long-term or acute care environments.
